"Unfortunately, CF is a progressive disease and even though I was doing everything 'right,' my lung function still gradually worsened," he said. "By the time I was 30, my lung function had declined to under 50%. By age 32, it was down to 35%, and by age 34 it was down to 22% and I was on oxygen 24/7.

"My doctors at Stanford had told me that I had only two years left to live unless I received a double-lung transplant. Initially, I was told that I would be on the transplant list for six months to one year. But miraculously, I received 'the call' — that they had a donor for me, only 2½ weeks after being added to the list."

That call came on May 15, 2012.

"It immediately changed my life for the better, in ways that I never thought possible," he said. "I'll never forget that first feeling of being able to breathe in deeply and clearly when I first woke up from the anesthesia and was extubated. It was surreal. For my entire life, since childhood, I had breathed with difficulty and it progressively worsened with time. Those first few breaths were so amazing to me. Even to this day, I don't take a single breath for granted.

"I also noticed other immediate changes, such as the coloration of my face, skin and fingernails, which looked pink instead of pale and blue. My lung function quickly rose to a stable 70%. I was in the ICU for only 1½ days. I was discharged from Stanford on Day 8, and I was back in the gym working out on Day 12. It was incredible. I definitely gained a new and unique perspective on life and I remain eternally grateful to my donor for the gift of life that I've been given."

Fight far from over

Unfortunately for De Fina, replacing lungs is not as simple as replacing a car battery.

"I was first diagnosed with acute rejection in 2013, only about one year post-transplant. The rejection never fully went away and it gradually worsened. By 2014, I had a diagnosis of chronic rejection, which is the worst grade of rejection you can have. This means your immune system has recognized that your transplanted organs are foreign and it is now attacking those organs, causing them to slowly fail.

"My lung function went from 70% down to 35% by 2017. I wouldn't say my life was truly in danger until about 2019 or 2020. The onset of the COVID pandemic definitely added another layer of stress to everything, but 2019 was when my lung function dipped back down into the mid-20's percentile range, and little things began getting very hard for me again, such as simply drying off after a shower, bending over to tie my shoes, or walking up stairs. I was back on oxygen 24/7, as much as 8 liters per minute, and required a Bi-Pap machine to sleep, used a Gastro-Jejunal feeding tube, and had a medi-port for frequent infusions."

De Fina underwent another double-lung transplant in 2020.

So far, it's been a perfect match.

"My lung function is currently the highest it's ever been in my adult life, at 80% and climbing," he said. "Because of this, I can exercise, play sports, compete, and do all of the things I love, including coaching volleyball at Justin-Siena, without any limitation. Since 2012, I've also done lots of motivational and inspirational speaking to share my story and speak on other topics, as well.

"I still take medications every single day, twice a day. I take a mix of anti-rejection meds, anti-fungals, anti-virals, antibiotics, vitamins, minerals, blood pressure meds, digestive enzymes, and a CFTR modulator for cystic fibrosis. But taking a handful of pills twice a day is way easier than trying to manage CF, especially in end-stage. My transplant has not only allowed me to survive, but it has revived my quality of life and allowed me to now thrive in ways that, not too long ago, I feared I'd never see again.

"The Bi-Pap machine, supplemental oxygen, GJ feeding tube — I weighed almost 30 pounds less than I do now — are now a thing of the past. I now sleep peacefully, I exercise and workout without limitation, I can eat anything I want, I can dedicate tons of time to the athletes I coach and move freely around the court, and argue with the volleyball referees —ha ha — without getting short of breath."

Makes Transplant Games debut

Even his first lung transplant made De Fina feel so young that he wanted to compete in sports again — with and against others who had gone through similar health challenges.

"I first learned about the Transplant Games of America not too long after my first transplant in 2012 from a dear friend of mine at Stanford who had her double-lung transplant about two years before me," De Fina said.

He wasn't able to attend the next TGA, held in 2014 in Houston, nor the 2015 World Transplant Games in Mar del Plata, Argentina.

But by 2016, his photopheresis treatments were spread out a bit more as his chronic rejection had become more stable.

"Despite my lung function hovering around 40%, I attended my first Transplant Games of America in Cleveland, Ohio," he recalled. "I competed for Team NorCal and came home with seven medals — two gold, two silver and three bronze. I was hooked. I won gold in the softball throw at track and field — what the TGA do in lieu of the javelin — and the 50-meter backstroke in swimming. I won silver in men's racquetball and the 4x100 relay in track. I won bronze in the long jump, the 200-meter sprint, and doubles tennis."

In 2017, De Fina attended the World Transplant Games in Málaga, Spain, and won a gold medal in men's basketball and a silver in men's javelin. He couldn't attend the 2018 TGA in Salt Lake City due to a scheduling conflict. He attended the 2019 WTG in Newcastle, England, but did not win any medals due to his declining health.

The 2020 TGA were pushed back to 2021 due to COVID and became "virtual" Games with participants completing various individual competitions locally and then submitting their verified results. De Fina won gold medals in the 50-meter freestyle swim, cycling 5K sprint, 100-meter sprint, 200-meter sprint, golf and the 5K run/walk.

The 2021 WTG were to be the first to be held on American soil, taking place in Houston, but were cancelled due to the pandemic.

But the TGA returned in 2022, in San Diego. De Fina attended them and came home with a record 13 medals — golds in the 4x100 track relay, 50-yard freestyle swim, 100 free swim, and 50 breaststroke swim; silvers for track in the 400 meters, 4x400 relay, long jump and high jump, and for pickleball in men's and mixed doubles; and bronzes in the 50 backstroke and for track in the 200-meter sprint and softball throw.

De Fina competed July 5-10 in the 2024 TGA in Birmingham, Alabama, and won nine more medals — golds in men's basketball, the 200 medley and 200 free relays in the pool, and in mixed doubles with pickleball partner Jill Nolen; silvers in the long and high jumps and softball throw; and bronzes in the 400-meter sprint and mixed 400 relay on the track.

De Fina has now won 37 medals in the Games, and he has no plans to stop.

"I definitely plan to compete in these Games again every time they're held, every two years, barring any unforeseen circumstances that prevents me from going," he said. "I've already started training for the WTG next summer in Dresden, Germany. I'm currently the team manager for Team CalNev, so I'll be pretty involved in the next TGA in 2026.

"What I take away from each and every TGA or WTG event is an enormous sense of gratitude to my donors for their gift of life, gratitude for the donors of all my transplant friends, and simply a deep, unique appreciation for every breath I take."

It brings him to tears at every competition.

"At every one of these Games, there's always a moment where I just break down," he said. "Sometimes it's in silence, sometimes by myself in my hotel room, sometimes in public, at the track or at basketball or on the medal stand. But there's always a moment where it just hits me and I think, 'Wow. I can't believe I'm doing this. Four years ago, I couldn't do anything even remotely close to this — I couldn't walk 10 feet, or up five stairs — but I just sprinted hard for 400 meters. Four years ago, doctors thought I'd be gone by now. But I'm still here, competing, and I'm wearing a gold medal around my neck.'

"I thank God for the miracles that have occurred in my life that have kept me here on this planet, allowing me to make memories with my family and friends. The TGA is such an inspirational event. Every time I go, I meet new people and witness such amazing stories of perseverance and fortitude on display. It's impossible to not be inspired."

De Fina said the TGA's mission is to spread awareness of organ donation and transplantation locally and nationally and show that transplantation works. The goal is to inspire people to register to become organ donors.

"Watching everyone who competes at the TGA and WTG events serves as a reminder that organ donation and transplantation doesn't just help people continue living, but it revives their quality of life and allows them to the thrive in ways that many never could before or thought would never be possible again," he said.

Family has been supportive

He and his wife, Denise, own De Fina Family Cellars in Napa. Its motto, breathe, refers to more than wine; it donates a portion of sales to charities benefiting cystic fibrosis patients. De Fina said his family and parents have all been "wonderfully supportive" with helping him manage his cystic fibrosis.

"Growing up with CF, my parents sacrificed a lot by dedicating so much of their time to taking care of me on a daily basis," he said. "There's a ton of stuff that goes into caring for a child with CF and trying to manage their disease. My parents traded shifts performing my chest percussion therapy — aka chest physiotherapy, which they were taught by RT's how to do — as well as my aerosolized nebulizer medication treatments, and sorting out all of my oral medications.

"They were both teachers and at least one of them had to take time off from work any time I needed to be hospitalized, which was a lot. In the first five years of my life alone, I was hospitalized over 50 times. Their dedication to my life as a child undoubtedly made a huge difference in my health. I have no doubt my CF would've progressed much faster if it weren't for them fighting for me.

"My wife and I met in 1998 at Fresno State and CF didn't scare her away. She loved me unconditionally, which is one of the many reasons why I knew she was the one. She jumped right in and wasn't afraid to learn all about CF or why I did these weird treatments and took all this medication. She must've thought I looked cute with a nebulizer mask on my face."

De Fina's father was a middle school P.E. Teacher, so Matt had access to every piece of sports equipment you could imagine.

"This allowed me to dabble in baseball, basketball, volleyball, soccer, tennis, racquetball, and track and field in elementary and middle school," he said. "In high school, I gravitated toward tennis, basketball, and track and field. Tennis became my chief sport and we won the league championship my junior year. I also did track and field for two years, competing predominantly in the pole vault, long jump, and sprints. I also played freshman basketball."

When he attended Fresno State, from which he graduated in 2001 with a degree in kinesiology, he played intramural basketball, softball and flag football through his fraternity.

Announcing games

De Fina started announcing Justin-Siena volleyball games a few years ago at the request of Assistant Athletic Director Andy Bettencourt.

"Andy initially asked me if I'd be willing to do it during the volleyball playoffs since he knew I wasn't shy on the mic and also knew that since I was a volleyball coach, I could quickly relay to the crowd who was credited with kills, assists, blocks, digs, etc.," he explained. "I agreed to do it and had a ton of fun with it, so he also invited me to announce our home games for basketball, as well.

"I'm a huge basketball fan, so, of course, I agreed and had a lot of fun announcing those games, too. I just basically looked at it like I was narrating the game for the crowd and tried to do some play-by-play stuff whenever I could. I tried to amp up the players by being animated on the mic whenever they'd hit a 3 or make a big play. I loved it.

"Then our varsity baseball coach (Jeremy Tayson) asked me to do his games, as well. So I was in the press box all spring announcing baseball. That was a ton of fun, too. I really appreciate our ADs and varsity coaches giving me that opportunity to be involved with the other sports in this way. I not only do it because I enjoy it, but mainly because, I hope, it makes the game experience more fun for the kids."

About the Games

Sandoz Pharmaceuticals — the predecessor to what is now known as Novartis Pharmaceuticals Corporation — and the National Kidney Foundation were the two organizations that in 1990 co-founded the U.S. Transplant Games, as they were called then. The first were in Indianapolis and drew about 400 participants, featuring celebrities such as Carl Lewis. Since then, the Games have been held biennially in different cities. By 2010, athletic participation had tripled to around 1,200 and total attendance had grown to about 5,000.

In 2011, the Transplant Games of America organization took over as the leader and organizer of the Games, with Novartis as the presenting sponsor. Since then, the TGA and Novartis have worked closely to raise awareness to the critical issues of organ donation and transplantation and bring it to the national stage.

"Their ultimate mission is always the same — encourage more people to register to as organ donors or become living donors, as well as showcasing through the athletic performance of our athletes that organ transplantation works, and it not only helps critical patients continue living, but rather, it completely revives their quality of life and allows them to regain their health and mobility in ways like never before.

"The TGA and WTG are both simply incredible, inspirational events that showcase amazing stories of perseverance and fortitude. Not only are there athletic events to witness for spectators, but the TGA and WTG also include donor memorials and tributes, Quarter-Century Club celebrations for those who are 25 years or more post-transplant, workshops, informational sessions, and tons of resources provided from vendors and sponsors.

"There are also competitions such as darts, cornhole, ballroom dancing, a singing and rapping competition called Lyrics for Life, trivia challenge, bowling, Texas Hold 'Em poker, and table tennis for those participants who don't wish to compete in the more rigorous sporting events. The Youth Olympiad is also a competition specifically for the youth participants. At our Games in Birmingham last month, our youngest participating athlete was 7 years old and our oldest was 89."

De Fina's Team CalNev, known as Team NorCal when it was founded in 1990, features athletes from Fresno to the Oregon border. The majority are from the San Francisco and Sacramento areas and are followed by medical centers such as those at Stanford University, UC San Francisco and UC Davis. It was renamed Team CalNev in 2018. Each TGA team has an affiliate Organ Procurement Organization as its primary sponsor and/or manager. Team CalNev's OPO is Donor Network West, based in San Ramon. The team represents Northern California and northern Nevada, mainly the Reno area.
